Myth: Annual Exams Aren't Necessary Without a Pap Smear

It's a common misconception that if you don't need a Pap smear, you can skip your annual gynecological exam. However, annual exams are essential for overall reproductive health. They include breast exams, pelvic exams, and discussions about menstrual health, contraception, and other concerns. Regular check-ups help detect potential issues early and keep you informed about your health.

Even when a Pap smear isn't needed, annual exams provide an opportunity to discuss any health changes, screen for infections, and assess your general well-being. Staying on top of your annual visits ensures you're proactive about your health, rather than reactive.

Myth: HPV Vaccinations Are Only for the Young

Another pervasive myth is that HPV vaccinations are only necessary for young people. While it's true that the vaccine is most effective when given before exposure to HPV, adults can still benefit. The HPV vaccine is the only vaccination that can prevent certain types of cancers caused by the human papillomavirus, making it an important consideration for ongoing health.

Myth: All Birth Control Methods Cause Weight Gain

Many people believe that all forms of birth control cause weight gain. In reality, the only method consistently linked to weight gain is the Depo-Provera shot. Other methods, including pills, IUDs, and implants, have not been shown to cause significant weight changes in most users.

Choosing the right birth control is a personal decision that should be based on accurate information and individual health needs. Discussing your options with a healthcare provider can help dispel myths and find the best method for you without unwarranted concerns about weight gain.

           Myth: Hysterectomy Means Removing Everything

There's a common misunderstanding that a hysterectomy involves removing all reproductive organs including the ovaries. However, a total hysterectomy means removing the uterus and cervix which is the part of the uterus in the vagina. A partial (or subtotal) hysterectomy consists of removing only the main part of the uterus leaving the cervix in place. In this case pap smears are still needed. This is only done in a few cases when it is considered too difficult to remove the cervix. The ovaries are usually left intact unless there is a medical reason to remove them. The ovaries will continue to function normally to produce hormones until menopause. Most hysterectomies can be done through small incisions making recovery much easier.

Clarifying the different types of hysterectomies is crucial for patients to understand their surgeries and the implications for their health. Knowing the difference helps set realistic expectations and informs post-surgical care and recovery.

Menopause Decoded – A Comprehensive Guide

Menopause is more than just the end of menstruation; it's a significant phase in a woman's life that brings various changes and challenges. While the average age of menopause is between 51 and 52, it can occur anytime from the early 40s to late 50s. In this guide, we'll delve into what menopause entails, its symptoms, and the latest treatment options, offering clarity and confidence for those navigating this transition.

           Understanding Menopause Symptoms

Menopause marks the end of a woman's reproductive years, occurring when the ovaries run out of eggs. The transition, known as perimenopause, can last one to two years, with symptoms like irregular periods, hot flashes, and mood changes.

           Symptoms and Concerns

The hallmark of menopause is hot flashes, experienced by almost all women. These can vary in intensity and frequency, often worsening at night and disrupting sleep. Other common symptoms include night sweats, mood changes, decreased libido, accelerated bone loss, and vaginal dryness.

           Hormonal Therapy and Alternatives

Hormonal therapy, a traditional menopause treatment, faced scrutiny after the early 2000s WHI study, particularly regarding Premarin and Provera. This led to a reevaluation of hormone therapy, highlighting the need for safer alternatives. Today, treatments like bioidentical hormones Estradiol and micronized progesterone offer effective relief with a favorable risk-benefit profile.

           Long-Term Management

The decision to start hormonal therapy is personal and should consider individual health profiles. Most women can continue hormonal therapy indefinitely, enjoying benefits like improved sleep and stronger bones. Misconceptions about bioidentical hormones often arise, but at Herrmann Practice, we provide clear, factual information to guide our patients.

           Special Considerations

Menopause's onset can be unpredictable, and in some cases, it may occur prematurely before the age of 40 due to factors like autoimmune processes or chemotherapy. Additionally, surgically removing both ovaries also induces menopause. Understanding these nuances is key to personalized menopause care.

           The Truth About Bioidentical Hormones

Bioidentical hormones, often surrounded by misconceptions, are as effective and natural as any other hormone products. Estradiol, for example, can be applied through the skin, offering a safer alternative to oral pills by minimizing blood clot risks.

Bioidentical hormones like Estradiol and micronized progesterone are FDA-approved and offer a safe, effective treatment for menopause symptoms. They are as 'bioidentical' as any hormones available on the market.

Herpes Simplex Virus, or HSV, is a prevalent virus with two distinct types: HSV-1 and HSV-2.

           HSV-1

: Often the culprit behind cold sores, it affects nearly half of the U.S. population over 50.

           HSV-2

: This sexually transmitted variant leads to genital lesions, impacting about 12% of the U.S. populace.

Many infected individuals remain symptom-free. Both types can lie dormant, reactivating when the immune system is weakened.

           Symptoms and Concerns:

Many individuals infected with HSV may never show symptoms. Both types can lie dormant in the nervous system, becoming active when the immune system is weakened. HSV-2 is particularly distressing due to its social implications and the risk of transmission to sexual partners.

           Treatment and Testing:

HSV can lead to painful open sores, with the initial infection often being the most severe. Medications are available to reduce the severity and frequency of outbreaks, but they cannot eradicate the virus. The primary medical concern arises if a pregnant woman has an active infection during delivery, as this can lead to severe infections in the newborn. In most cases, a C-section can prevent this transmission.

           Common Queries:

A frequent question posed by patients is the duration they've had the virus before symptoms appear. This varies as everyone's immune system responds differently. Testing is most effective during an active outbreak, with a swab of the lesion detecting the virus. Blood tests, which detect herpes antibodies, indicate exposure but not necessarily an active infection.

HPV, or Human Papilloma Virus, is the main cause of cervical cancer. It's notably prevalent among sexually active women under 30, with some estimates as high as 80%.

Treatment and Prevention:

While there's no specific medication for HPV, the Gardasil vaccine offers a shield of protection. It's crucial to get vaccinated before exposure. Most patients' immune systems will eliminate the HPV virus by age 30.

HPV can be detected on a pap smear. However, due to its high prevalence among women under 30, routine testing isn't standard for this age group.

The Gardasil vaccine protects against strains of HPV that cause cervical cancer and genital warts. Pediatricians recommend the vaccine for both boys and girls starting at age 9. A common question is the rationale behind such early vaccination. The answer lies in the enhanced immune response in younger children. Before age 15, only two doses are required. The primary side effect is soreness at the injection site, but it's a small price to pay for a vaccine that can prevent cancer.

Genetic testing plays a crucial role in unraveling the secrets hidden within our genetic code, shedding light on predispositions to certain types of cancer. Through comprehensive analysis, these tests can identify specific gene mutations that increase the likelihood of developing colon, breast, or ovarian cancer. In this blog post, we will explore the significance of genetic testing, its implications for early detection, and how you can benefit from our advanced testing options.

Unlock the Secrets of Cancer Risk

While many cases of cancer occur sporadically, without any known hereditary link, a significant portion is caused by genetic factors passed down through generations. By identifying these gene mutations, healthcare professionals can assess your risk level and develop tailored strategies for prevention, early detection, and treatment.

It's important to note that genetic testing is not meant to be a definitive diagnosis of cancer. Rather, it serves as a valuable tool for assessing your risk and guiding your healthcare decisions. If your test results indicate an increased risk, further steps can be taken, such as increased screening, preventive measures, or discussions about potential treatment options.

Is Genetic Testing Right for You?

Determining whether genetic testing is appropriate for you involves assessing certain criteria and understanding the potential benefits. If you have a first-degree relative (such as a parent, sibling, or child) who was diagnosed with colon, breast, or ovarian cancer before the age of 50, or at any age for ovarian cancer, you may be eligible for genetic testing. These tests focus on identifying gene mutations that can increase your susceptibility to cancer development.

Genetic testing not only provides valuable information about your own cancer risk but also has implications for your family members. If a gene mutation is identified, it means that other family members may also be at risk. Therefore, genetic testing results can empower your entire family to make informed decisions regarding their healthcare.

How Gene Mutations Influence Cancer Risk

Gene mutations are changes or alterations in the DNA sequence that can disrupt the normal functioning of genes responsible for cell growth, division, and repair. Some mutations can increase the likelihood of developing cancer by interfering with the body's ability to control cell growth and prevent the formation of tumors.

Genetic testing analyzes specific genes associated with cancer predisposition, such as BRCA1 and BRCA2 for breast and ovarian cancer, and Lynch syndrome genes for colon cancer. By detecting mutations in these genes, healthcare professionals can assess your risk level and develop targeted strategies for prevention, early detection, and treatment.

It's important to note that not all gene mutations lead to cancer, and having a mutation does not guarantee the development of the disease. However, identifying certain mutations can significantly increase your risk, enabling you and your healthcare provider to take proactive steps to mitigate that risk.

The Power of Genetic Testing in Early Cancer Detection

The power of genetic testing lies in its ability to arm individuals with knowledge and empower them to take proactive steps towards early cancer detection. By identifying gene mutations associated with an increased risk of cancer, genetic testing becomes a vital tool in the fight against this devastating disease.

Genetic testing allows you to be proactive rather than reactive when it comes to your health. Armed with the knowledge of your genetic risk factors, you can work closely with your healthcare provider to establish a personalized screening regimen. Studies have shown that individuals with certain gene mutations have up to a 70% higher risk of developing breast cancer and up to a 50% higher risk of developing colon cancer. This knowledge enables you and your healthcare provider to implement more frequent screenings or specific tests designed to detect cancer at its earliest stages when treatment options are often more effective.

For instance, if you discover that you have an increased risk of developing breast cancer due to a specific gene mutation, research has demonstrated that increased surveillance through regular mammograms and MRIs can improve early detection rates by up to 40%. Additionally, discussions about preventive surgeries, such as prophylactic mastectomy or oophorectomy, have been shown to reduce the risk of developing cancer by up to 90%. Lifestyle modifications, such as adopting a healthier diet and exercise routine, can also play a significant role in minimizing your cancer risk.

Patients on hormonal birth control may experience breakthrough bleeding, which can usually be treated with a cycle or short course of oral contraceptives. Patients should be aware that breakthrough bleeding is common and not a cause for alarm. Patients not on hormonal medications should have a menstrual period every 21-35 days and excessive flow may require further evaluation for uterine pathologies. Heavy or prolonged periods can be treated hormonally or surgically, including the use of hormonal IUDs or endometrial ablation.

STI testing is offered to all sexually active patients and recommended according to CDC guidelines for Chlamydia, Gonorrhea, Syphilis, and HIV. Dr. Herrmann's team also tests for ulcerated vulvar lesions and provides partner treatment per the patient's request.

Dr. Herrmann's office provides testing and treatment for common vaginal infections, such as bacterial vaginosis, monilial vaginitis (yeast), and trichomonas. Dr. Herrmann and his team work with patients to provide the most effective and appropriate treatment for their specific condition.

Bacterial Vaginosis (BV) is the most common, characterized by a fishy odor and whitish/grayish discharge. Treatment options include metronidazole orally or generic metronidazole gel. Recurrent/chronic cases may require longer treatment with Boric acid capsules. Monilial vaginitis (yeast) causes pruritus and a cottage cheese-like discharge, treated with Diflucan. Trichomonas causes a bubbly discharge, treated with oral metronidazole. Partners can be treated if desired.

The Herrmann Practice provides guidance and treatment for menstrual disorders, including breakthrough bleeding for patients on hormonal birth control, heavy flow, and prolonged periods. Dr. Herrmann and his team also evaluate uterine pathologies, such as uterine fibroids and endometrial polyps, and provide hormonal or surgical treatment, including hormonal IUDs and endometrial ablation.

Menopause typically occurs between 45-55 years of age and is characterized by irregular menstrual cycles and hot flashes. After 12 months of no periods, any bleeding should be evaluated. Hormonal therapy (HT) can be considered for patients experiencing severe symptoms, with estrogen available in various forms and deemed safe for most patients. Progesterone is required for patients with a uterus. HT can provide additional benefits such as preventing bone loss and improving skin and mood. Bioidentical patches and oral progesterone are recommended as the safest HT options.

           Vulvar Pruritus

Vulvar itching is a common condition that can be caused by a variety of factors such as yeast or fungal infections and chemical irritants. Vulvar skin is particularly sensitive, especially for those with lighter skin. It's important to avoid fragrance-containing products and laundry detergents, as they can exacerbate itching and burning. A steroid ointment can provide relief and help break the itch-scratch cycle. Over-the-counter products like Vagisil may worsen symptoms. Don't hesitate to seek help and treatment from a healthcare provider for vulvar itching. We are here to help diagnose and treat these issues, and there is no need to feel embarrassed or miserable.

           Pap Tests

Pap tests are an effective screening method for cervical cancer and can detect abnormal cells long before they become cancerous. The test also detects vaginal infections and STIs. For patients over 30, the test is used in combination with an HPV test. The HPV vaccine is safe and effective in preventing HPV-related cervical cancer, but should be given before exposure to the virus. An abnormal Pap test does not necessarily mean cancer and requires further evaluation in the office.

           Breast Lumps

Finding a breast lump can be extremely alarming, but most lumps are not cancerous. We can see any patient who feels something abnormal in their breasts on the same day. After an exam, we can determine if further evaluation with a mammogram or ultrasound is necessary. We recommend annual mammograms starting at age 40, as they are the best method of screening for breast cancer.

           Urinary Tract Infections

Urinary tract infections (UTIs) are common in females and are caused by bacteria from the rectal area. Symptoms include burning with urination, frequent urination, and a constant urge to urinate. It's important to empty the bladder before and after intercourse. Untreated UTIs can develop into kidney infections and require hospitalization. We can usually treat UTIs in the office. It’s best to obtain a fresh specimen in the office for culture.

Urine leakage can occur in women of all ages and may worsen with childbirth and aging. It can be caused by changes in anatomy or weakening of the bladder's support. Treatment options include exercises, medication, and surgery. Don't suffer in silence, we can help. Referral to a urogynecologist may be necessary in some cases.

Over 9 million women in the U.S. get an STI each year. Sexually Transmitted Infections (or STIs for short) have been on the rise amongst many communities in the U.S. since 2014, and affect women of all ages. However, women who fall in the 18-25 age range are found to be the most susceptible subset of females at risk of contracting an STI. Even if you believe “it would never happen to me,” the truth is that anyone who is sexually active is susceptible to these infections. Dr. Herrmann and the entire staff at The Herrmann Practice care about your safety. We’re dedicated to providing ALL our patients with comfortable and compassionate care as well as supplying the knowledge and resources to help protect you and your body. The CDC recommends getting tested for STIs every 3-6 months, however it would be beneficial for anyone who’s sexually active to do regular testing monthly, especially if you have multiple or anonymous partners. Here are the 3 biggest reasons that you should get STI testing done regularly and how The Herrmann Practice can help.

Asymptomatic means that they do not show symptoms, and you might not even know if you have it. Because of this reason, it’s better for your sexual health to do regular STI testing. Just because you do not have symptoms does not mean you cannot spread an STI to your partner. If you’re worried you might have a latent STI, or if you’ve seen suspicious bumps around your genital areas, it’s better to be safe than sorry! Herpes, for example, affects

As of December 2021, Omicron has been detected in most states and is rapidly increasing the proportion of COVID-19 cases it is causing. The Medical Professionals at The Herrmann Practice are here to put your mind at ease. Read our blog to discover 8 things you should know about the Omicrom Variant!

1) The Omicron variant of Covid 19 is extremely contagious. If you are unvaccinated you are highly likely to become infected.

2) The symptoms are different from Alpha or Delta. It is mostly an upper respiratory infection causing a scratchy throat and runny nose like a cold plus fatigue. Most patients don’t have a fever or lose their sense of taste and smell. Some people have no symptoms at all, but can still infect others. While Omicron is less severe in most people it can still cause serious infections, hospitalizations, and death.

3) Vaccinated people can become infected, but most are protected from serious infections, hospitalization, and death.

4) Boosters are currently recommended for everyone over the age of 18 - five months after Pfizer and Moderna and two months after Johnson and Johnson.

5) The current tests are able to detect Omicron in about 80% of cases with the home tests and over 90% with the PCR test.

6) The time from exposure until a positive test or symptoms is shorter, approximately five days instead of ten. Therefore the quarantine period has been reduced to five days followed by five more days of constant mask wearing and social distancing.

7) Cloth masks are minimally effective, surgical masks are better, and N-95 are best.

8) Vaccination with Moderna and Pfizer are highly recommended and a booster after five months. If you don’t want to get sick or spread Covid, get vaccinated and boosted. These vaccines have proven to be tremendously safe with minimal side effects.

          What is a mammogram?

A mammogram is a very low dose X-ray picture of the breasts that doctors use to look for early signs of breast cancer. While you may feel anxious or nervous before your mammogram, the screening process is very important for your health maintenance. No matter your stage in life, it is common to have questions and concerns about your womanly health. At the Herrmann Practice, we are here for you. That's why we’ve compiled a list of frequently asked questions we receive to help you better understand what’s to come in your next mammogram!

            FAQ:

Q: At what age should women start having mammograms?

A: The standard recommended starting age is 40 and then annually.

Q: What if I am high risk for breast cancer, should I have a mammogram before age 40?

A: High risk primarily means women with the BRAC 1 and BRAC 2 genetic mutations and women that have already had breast cancer. These women need close surveillance with mammograms and possibly MRIs.

           Q: Are mammograms painful?

A: Most women describe a mammogram as mildly uncomfortable. For women with very large or sensitive breasts it may be more uncomfortable.

Q: If I have breast implants, can a mammogram cause them to rupture?

A: This would be an extremely rare occurrence.

Q: What is the difference between a screening mammogram and a diagnostic mammogram?

A: A screening mammogram consists of two views. A diagnostic mammogram is a more focused mammogram consisting of three views. A diagnostic mammogram is medically indicated when there is an abnormality on the screening mammogram or a lump is present. A radiologist is also present for a diagnostic mammogram to do an exam and be sure the mammogram is focused on the abnormal area.

Q: What if I want a diagnostic mammogram, can’t my doctor just order that?

A: It must be medically indicated or insurance companies won’t pay for it.

Q: Can the radiation from a mammogram be harmful?

A: No, the amount of radiation from a mammogram is less than that of a chest x-ray and does not increase the risk of breast cancer.

           Q: What is a 3D mammogram?

A: The technique is the same but instead of just one view there are multiple views.

           Q: Can anyone request a 3D mammogram?

A: Yes but not all insurance companies pay for them unless you have dense breasts.

           Q: What are dense breasts?

A: Breasts are made up of glandular and fatty tissue. Women who have a higher ratio of glandular tissue are said to have dense breasts. This has nothing to do with firmness.

           Q: How accurate are mammograms?

A: The current mammogram equipment is very good (of course not perfect) at detecting breast cancer before a lump can be felt. Mammograms are less accurate in women with dense breasts and women with breast implants. In some cases the mammogram may be inconclusive and requires a diagnostic mammogram, an ultrasound, and in some cases a biopsy. Sometimes a six month follow up mammogram is recommended.

Q: Does it matter where I get my mammogram done?

A: Any mammogram is better than not having one. Mobile units that used to go to workplaces were popular due to convenience. These no longer exist for a variety of reasons. A hospital or breast center generally has the latest equipment and expert radiologists.

Q: Is there an age when I can stop having mammograms?

A: That depends on your overall health. If you have a life expectancy of less than ten years a mammogram probably isn’t of much value.

            Conclusion:

When should young women start seeing an OBGYN? Everyone’s health care needs are different, but establishing women’s healthcare during adolescence is important to promoting long-term healthy lifestyles. In fact, according to the American Congress of Obstetricians and Gynecologists (ACOG), it is recommended that young females have their first Ob-Gyn visit when they’re 13 to 15 years old.

           Establishing a Healthy Lifestyle

It is not at all uncommon for girls to start experiencing embarrassment about changes after puberty. Many preteens and teens have difficulty opening up about such topics with parents and other adults. An initial visit with an Ob-Gyn may only be a talk with the women’s healthcare provider with no exam, however, if a young woman has become sexually active, contraception and STI testing is important.

It is important to establish a relationship with a trusted medical professional to make continued medical conversations more comfortable when sensitive topics come up. Additionally, it is important to promote a healthy lifestyle through regular annual women’s health exams at an early age to prevent any health complications, provide contraception, and in some cases, test for STDs like gonorrhea, chlamydia, syphilis, and HIV.

What is the best way to treat my period cramps?

Cramps can be treated safely with over the counter medications such as Advil, Tylenol, and Motrin. For more severe cases, Oral contraception or ‘the pill’ is not only a great birth control but makes your menstrual blood less heavy, which minimizes your cramps each month. Other remedies may include a warm bath, heating pad, or a hot water bottle in addition to exercising regularly, eating a healthy diet and avoiding salt, sugar, caffeine and alcohol.

It is important for women to give themselves regular breast exams to check for any changes or lumps that may indicate medical issues.

Yes, you can have sex on your period. The risk of getting pregnant is still present during this time. It is best to practice safe sex practices no matter when you elect to have sexual relations.

It’s no secret that Gynecologist’s offices have long waits, both in the office and scheduling, as many practices have difficulty working patients into their schedules for urgent problems. In some cases, it may take weeks to get a routine appointment in many OBGYN offices. This is mostly due to the demands of busy obstetrical practices and overbooking.

At The Herrmann Practice, we strive to ensure our patients’ health with the ability to provide care quickly, in most cases the same-day. Since our office has shifted its focus away from obstetrics, we are able to offer same-day appointments for urgent problems and common women’s concerns, such as:

            Breast Lumps:

If you notice an usual lump in your breasts during a bath or shower, contact our office immediately for an examination.

Our office provides same-day screenings to ensure your health with a mammogram to identify if there are signs of breast cancer.

           Menopause Complications:

If you are experiencing uncomfortable symptoms from menopause, we can help. Our practice offers a variety of hormonal treatments that can help you feel at your very best and overcome the negative effects of menopause.

Bladder or Urinary Tract Infections (UTIs):

If you notice a sudden onset of painful urination, more frequent urination, and constant urges to urinate. If possible it is best to get a urine sample in the office for a culture. We are seeing more bacterial strains that are resistant to the usual antibiotics. Early treatment is important to prevent the symptoms from getting worse and leading to a kidney infection.

Cranberry juice and over the counter medications are not adequate treatments for UTIs.

           Vaginal infections:

These can be very bothersome to women with common symptoms such as discharge, odor, itching and burning.

The most common vaginal infection Is bacterial vaginosis, which is caused by certain bacteria that overpopulates the vagina causing a fishy odor. This is also the most difficult to treat and can become a recurrent or chronic infection in many women. This can be treated with a prescription vaginal gel or oral antibiotics. Some patients will require longer term treatments.

While over the counter treatments of vaginal creams are very effective for yeast infections. Creams and even suppositories are somewhat messy to use and the majority of vaginal infections are not caused by yeast.

If you are unsure what type of infection you have or have tried a non-prescription cream or suppository and it didn’t work, you need to make an appointment to get an accurate diagnosis and the proper treatment.

With the recent onset of the Delta Variant of Covid-19 throughout the United States, it is more important than ever to get the Vaccine to help eliminate the threat to the overall public. The longer it takes for everyone to get vaccinated, the more risk there will be to the public from the Delta Variant and any other future mutated strains of the virus. It is without a doubt that the Delta Variant is more contagious than previous strains, which poses an even greater public health risk.

Currently, there are many questions and concerns about Covid vaccinations and their safety. Many people are concerned that because of their rapid development, safety may have been compromised. As a Medical Professional, I have read extensively and studied the process of the development of these and other vaccines. I am confident that all vaccines approved by the FDA are safe and effective. The safety trials were the same as for all other vaccines currently on the market. In fact, research for coronavirus has been going on for over 10 years.

Effects of Covid-19 Vaccine on Pregnant Women:

The Covid-19 Vaccines approved by the FDA are all safe for use in pregnant women and those who are breastfeeding, however, there is still little data on this subject.

There are currently Clinical Trials that look at the safety and how well the COVID-19 vaccines work in pregnant people underway or planned.

Vaccine trials in animals before or during pregnancy found no safety concerns.

There are new studies that show that immunity to the Covid-19 Virus may pass along to newborns if the mother has been vaccinated.

No matter what stage of life you are in, the experts at The Herrmann Practice are here to ensure that you are living a healthy life. Vaccines have the potential to eradicate diseases as we have smallpox and nearly polio in the past. However, this will only happen if 70 to 90% of the world’s population gets vaccinated.

All vaccinations have small and possibly unknown risks and side effects. When it comes to the Covid-19 Vaccine, the trials have demonstrated that the benefits far outweigh the risk of getting Covid and possibly having long term disabilities or dying from it. Unfortunately there are always people that can’t or won’t get vaccinated. The people that can, but won’t get vaccinated, put the people that can’t get vaccinated at risk. I feel that healthy people that can get vaccinated have a moral obligation to get vaccinated, not just for themselves, but also for our more vulnerable populations.
